At least two reviews are underway after a Fraser Health-contracted ambulance mistakenly delivered the wrong patient to a Surrey, B.C., home just before Christmas.
Sunny Hundal was at Surrey Memorial Hospital on Dec. 22 with his bedridden father, waiting for a patient transfer. Around 6:30 p.m., his wife discovered that a hospital transfer had arrived at their home with a different patient—an elderly man in a hospital gown who appeared confused and anxious.
The error occurred because the two patients shared the same last name and first-name initial, and the transport crew failed to verify the full first name and Personal Health Number before leaving the hospital. The patient was quickly redirected to the correct home.
Hospital Transfers confirmed immediate steps have been taken to reinforce full patient identification verification, provide refresher training, and review procedures with Fraser Health. Fraser Health stated all hospital discharge steps were followed and that both patients were safely returned to their homes.
Sunny Hundal said his father was eventually transferred home safely and emphasized he does not blame hospital staff, noting the busy, rainy, and chaotic conditions at the time.
